Intake Form: A brief medical and bodywork history. The questions on this form allow me to prepare for our session together. While everything may not apply to you, the more information you provide, the better
Photo and video release: Before and After photos may be taken to show your progress, or short video clips may be recorded for demonstration purposes. These photos and videos may be posted on this website. You may choose to have your face blurred or covered when posted.
Cancellation Policy Acknowledgment : Please sign and acknowledge that you understand and accept the cancellation policy.
Credit Card Authorization : Please complete with your preferred credit card which will be used to secure appointments scheduled, as well as in the event there is a cancellation within 24 hours (or no show). Please let me know if you'd like to use this method of payment for your sessions, or if you prefer to use another method when we meet.
